AHP is excited to welcome Sam Baron to the team in the role of data coordinator. Sam recently graduated from D’youville College with a Bachelor’s degree in Health Service Management. Sam will be supporting the care management team with centralized data support and also work with some of our community practices to assist with population health initiatives, closing gaps in care, and improving our quality measures.
