The Collaborative Care Model – Behavioral Health Support for Your Practice

Given the significant difficulty with access to mental health resources in the region, we are happy to announce that AHP will soon offer a model of integrating behavioral health support throughout our network. Through a partnership with Concert Health, we will be implementing an evidence-based model of behavioral health (BH) integration which has been proven to effectively improve patient outcomes and decrease the burden on primary care practices.

The Collaborative Care Model of BH integration assigns a mental health therapist to your practice to work remotely in support of your patients and providers. It also assigns a psychiatric provider to review cases with the therapist and make additional recommendations, including medication management. Although participating in this initiative will require some time commitment from the practice, it will allow you to bill for services provided and retain a portion of the revenue. AHP is excited about introducing this level of support to practices that choose to participate. We are already having conversations with practices around the Collaborative Care Model and look forward to future expansion.
