2016 PCMH Program Applications Now Available
May 25th, 2016 | Network NewsApplications are now available for AHP’s 2016 Patient Centered Medical Home (PCMH) Project. If your adult PCP or pediatric practice would like assistance in achieving NCQA PCMH recognition, consider applying for valuable consulting expertise in the practice transformation process. Practices accepted into the program will work closely with RMS Healthcare to assess the practice’s readiness for PCMH recognition, develop a roadmap for practice transformation, and facilitate the process of achieving practice transformation and preparing the practice’s submission for recognition under NCQA 2014 standards.
