Options for Participating in the NYHER 1115 Waiver Program
April 4th, 2025 | NewsThank you for your involvement in the recent AHP/FLIPA SCN forum and your commitment to advancing social care and promoting health equity. As we continue to refine the details and processes for this project, we will keep practices updated on any important developments.
As discussed in the forum, AHP is coordinating three options for practices to engage in the enrollment, screening, and management components of this initiative. Below are the available participation options for your reference:
- Option 1: No AHP Involvement – Practices may choose to contract directly with FLIPA or opt out of participation altogether.
- Option 2: Shared Responsibility – Practices will be responsible for screening patients using the AHC tool, while AHP will handle care navigation and management for eligible patients.
- Option 3: AHP Full Support/Connection to SCN – AHP will conduct the AHC screenings, enter data into the SCN platform, determine eligibility, and manage care navigation and management.
