AHP Pharmacy Efforts Recognized by HANYS

The Healthcare Association of New York State (HANYS) recognized Accountable Health Partners and the University of Rochester Medical Center for their collaborative work to implement comprehensive medication management (CMM) as a model in primary care settings. Through the program, clinical pharmacists on the Primary Care Clinical Pharmacist Team were embedded into primary care practice sites. They used the CMM model of care to optimize medication regimens and improve disease control. AHP’s Clinical Pharmacists, Jennifer Radcliffe, PharmD, and Katherine Diehl, PharmD, supported this work as the embedded pharmacists at Alexander Medical Group and Internal Medicine of Brighton, respectively. Over 19 months, the Primary Care Clinical Pharmacist Team engaged with 716 high-risk patients, resolving more than 3,500 medication therapy problems. Read the HANYS Member Spotlight here.
