Attention AHP Specialists: Co-Management Agreements and PCMH
July 29th, 2016 | ArchivePatient Centered Medical Home (PCMH) is a model of care that emphasizes team-based care and improved care coordination, and AHP has supported primary care efforts to obtain PCMH recognition since our consulting program began in 2014. One of the standards for PCMH requires primary care practices to enter into co-management agreements with the consultant practices that provide specialty care to their patients. The purpose of a co-management agreement is to outline expectations for communications between the PCP and specialist practices in order to maximize care coordination between providers.
AHP PCP practices that are pursuing PCMH are starting to send these requests to the specialists that most frequently care for their patients. Agreeing to enter into a co-management agreement can help improve communications between practices, while also communicating to your referring PCPs that you value their referrals and support their PCMH efforts.
