Data Coordination Efforts Showing Promise
April 29th, 2019 | ArchiveData coordination began at AHP in 2018 with the goal of supporting community-based primary care practices with closing gaps in care by providing access to data residing in the EMR or Arcadia dashboard for clinicians and other team members to act on. Pre-visit planning, outreach efforts and measure-specific reporting are all supported by data coordination efforts.
AHP’s data coordinator, Sam Baron, has been working in three practices where his role has been customized for each, in recognition of each practice’s unique needs, operations and practice populations. Sam’s specific duties have included identifying needed patient referrals, outreaching to specialists to secure documentation of gap closure, and working with the practice’s internal team to ensure patients have follow-up appointments to address open gaps. Through Sam’s efforts, we better understand the importance of integrating data coordination into the care team and the value of close alignment with care management. Leveraging these partnerships provides the data coordinator with the context necessary to more precisely support gap closure for individual patients.
