AHP Improves Fall Risk Performance

One of the quality measures AHP’s Optimization Team focused on in 2018 was fall risk. Since the fall risk measure is one that relies heavily on correct documentation, our IT Project Director Elizabeth Sharp, worked with six pilot practices to develop EMR-specific templates that would accurately capture when a fall risk assessment was done. The templates allowed practices to close gaps in the measure and increase the number of patients screened, as well as create an appropriate plan of care. As you can see in the graphic, one practice moved from below the 25th percentile to the 50th, another from below the 25th to the 75th, and three practices went from below the 25th percentile to the 90th percentile. In total, 2,132 more patients were screened for fall risk in 2018 when compared to the same time period in 2017.
