Population Health Investment Fund Proposals: Deadline Extended
September 27th, 2018 | Network NewsEarlier this month AHP announced the Population Health Investment Initiative to provide financial support for projects that improve value-based care delivery. Providers, care managers, allied health providers or any team working within the AHP network are invited to submit brief proposals that describe the initiative, its impact on cost, quality, or both, and a brief budget for the amount being requested and how that money will be used.
Due to the surge of inquiries within the last week of members interested in submitting a proposal for the Population Health Investment Fund, we are extending the submission deadline to end of day on Friday, October 5, 2018.
