Readmissions Reduction Pilot Performance

In an effort to reduce all-cause readmissions by 15%, AHP embarked on a pilot program with Highland Hospital earlier this year.  Partnering with one Highland inpatient unit, the pilot bridges our inpatient transitional care management and pharmacy efforts with our outpatient care management strengths through a series of “6 Touches” that supports patients upon discharge, provides accurate information about their current medical status and reconciled medications and reduces decompensation and readmission.

Early data show a positive improvement in readmission rates, however we have a long way to go to reach our goals for patient inclusion to test this program. We anticipate this pilot will run through the end of 2019 with plans to make a final report on pilot outcomes by February/March of 2020. If successful, we anticipate a wider roll-out, and we will keep you informed of our progress and success for this very important initiative. We greatly appreciate the engagement from across the spectrum of the network, and we look forward to sharing what we’ve learned about enhancing the care of our patients during this vulnerable transitional period.
