2025 PCP Reward Plan- Quality Measures
February 28th, 2025 | Quality•NewsAHP is dedicated to continuously improving patient care, and our quality measures reflect this commitment. After successful negotiations with payers, we have finalized our quality metrics for 2025. These measures, which focus on the health of both adult and pediatric populations, will be shared with our network, along with detailed communication plans for monitoring performance. The only new addition is the Diabetic KED measure. This update will also include information about our transition to Health Catalyst, our new population health management platform, as well as a comprehensive review of the components of the PCP Reward Plan.
AHP has developed several resources to help providers meet our cost and quality goals, including:
- Well Child Roadmaps: An excellent resource for parents of young children that highlights the importance of well-child visits and comprehensive screening.
- Pharmacy Pearls: Easily digestible newsletters covering various pharmacy topics to help providers navigate shortages, prescription switches, and best practices for medications related to specific disease states.
- Provider Engagement Report: A dynamic, electronically delivered tool that allows providers to access a holistic view of their Reward Plan performance.

The 2025 quality measures are included in the PCP Reward Plan-AHP’s incentive plan that rewards providers for their efforts to improve the value of care.
