Adult Population Health Dashboard – How Are We Doing?

In looking at our enterprise population dashboard and I am so impressed with how much effort and responsive care our PCP practices at AHP are providing to all of the patients of our network! As the nation is acclimating to our renewed focus on quality and population health we also are seeing the need to do even better to keep our quality percentiles in the same ranges they have been in the past.
For this iteration of the report we are noticing that the diabetic control rates and the nephropathy metric have fallen into a lower percentile. This change gives us another opportunity to review our current metrics and work with our care teams to try and reclose these previously closed gaps.
